I will have family, a partner or friend staying at the hotel during my stay (additional work colleagues attending the conference itself will need complete a separate registration).

I certify that I am authorized by my company to send in this registration form and by sending this form, I have agreed that Innovation Transportation Ltd T/A Centrolene Network will be booking my above registered company representative(s) with the conference center. Upon receiving Centrolene Network AGM 2022 confirmation, full payment is required. Cancellation Policy: Members who have confirmed through the registration form that they will attend the conference will have up until 2nd September 6pm Hong Kong Standard Time to cancel, at which time their Conference Fee’s will be refunded in full. An Administration fee of USD40.00 will be deducted from their conference fees when the refunded amount is transferred. Subsequently those that have not made full payment of Conference Fees by 2nd September will not be confirmed as an attendee and the hotel will be notified and any accommodation cancelled. Members that cancel their Hotel booking will be subject to the cancellation policy of Phuket Marriott Resort & Spa, available on their website.
